Ghouls are kind of a unique western monster where as ayakashi are a much more broad name.

my impression is that the evil damiyo sold his baby's parts to a bunch of demons for power
she may not have been one of those demons and dororo was about to make a mistake in killing her

It kind of leaves me with some questions though.
I was under the assumption there was a sort of concord between the demons and Hyakkimaru's body parts where each one had one of his parts.
So letting spiderlady go like this means he would never get a part of his body back.
But maybe it's just a simpler, kill any ayakashi and you'll get a body part lottery ticket.
I would say the way the series has set it up so far though, it was kind of indicating more the prior though.

Well your impression is exactly what we saw, which is what I meant with how the series had set it up that each body part was with a specific demon as opposed to sort of a random reward thing.
So yeah, maybe he's also been going around killing "ordinary" demons too on top of the ones that actually have his body parts.
After all if you don't know who's got your drops, the most certain way of getting them is just going around killing all the monsters on the drop table.

This still doesn't solve the "what if" of if the spiderlady has one of his body parts or not though.
